今天我们做一次标题党，所谓“大力神丸”就是通常的万艾可。
一位年轻小伙子主诉2月前服用一次泰国版西地那非后出现双眼视物模糊来诊。
什么，我一下子就联想到了西地那非导致的AION。此前我们也报道过不少服用“壮阳药”后出现AION患者，但多为中老年男性，难道这位年轻人有明显的解剖学拥挤视盘？
然而患者双眼视盘边界非常清晰，不符合AION的诊断，而且绝非高危视盘！

所以，定位于视网膜，很快其他检查结果出炉。
视野散在暗点，也并非AION
细心观察OCT发现些许端倪，双眼黄斑上缘的外核层似乎存在小片高反射病灶（红圈），由此我们联想到一种好发于年轻近视人群的疾病——急性黄斑神经视网膜病变(Acutemacularneuroretinopathy,AMN)。
AMN也是AZOOR家族一员，病因目前倾向由黄斑区视网膜深层血管网缺血造成。由于发病时间过长，错过了急性期，我们煞费苦心也只是发现精准多焦ERG找到黄斑中心区功能障碍的证据。
想当年辉瑞公司为了明确他们的蓝精灵和NAION发病是否有关而斥巨资发起的临床对照试验，虽然证明了自己的清白，但也是花费了不少费用的！
说回到我们患者所服用的药物，google了一下小袋子上Kamagra，发现原来竟然是印度产神药，并非FDA通过的万艾可（西地那非），而是非法药物！！！
所以，这锅西地那非应该不用背，甚至文献中报道的那例视网膜病变也是用了网购药物。不知道这里面是否还掺杂了其他成分？小伙子很爽气地给我留了一包药，看看群里面哪位大神可以帮助测测成分啊！
最终诊断：双眼AMN。
